The Local Coordination Service supports the Council’s vision and strategy for Adult Social Care and Health by working more effectively with the Voluntary and Community Sector, to build personal, community, and system resilience, underpinned by strengths-based practice which empowers people to remain independent for as long as possible. 1.2 The Local Coordination Service offers a person-centred provision with a strengths-based approach, focused on prevention and early intervention to help residents lead lives with greater control and independence. The Local Coordinators are embedded within the community, working closely with Adult Social Care and the Voluntary and Community Sector. They have an in-depth knowledge of local opportunities and networks and play a key role in helping people to identify and access this support. They also play a crucial role in identifying gaps in the community offer. The service promotes health and wellbeing, reduces isolation, helps people connect with their community and promotes independence, ultimately preventing, delaying, or reducing the need for formal care.
