Description of the procurement

Lambeth Clinical Commissioning Group (CCG) are procuring a community diabetes service consisting of the following elements:
• Referral management
• Advice and guidance for patients and healthcare professionals
• Community Clinics for type 2 diabetes care
• Self management support and structured education provision for patients at high risk of developing diabetes, newly diagnosed or with pre-existing diabetes
• Virtual clinics in primary care
• Education and learning for primary and community care
