Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers

List and brief description of conditions:  NHS Lambeth Clinical Commissioning Group is looking to commission a direct access, non-urgent and non-obstetric community gynaecology ultrasound service. The service is scheduled to commence 1st April 2018.
The service will be provided for patients 18 years and above who are registered with a Lambeth GP. Services will be offered from three mandated sites which will be geographically distributed across the three localities of the borough:
•South West Locality: Gracefield Gardens Health and Social Care Centre, Streatham, London SW16.
•South East Locality: West Norwood Health and Leisure Centre, 25 Devane Way, West Norwood, London SE27 0DF.
•North Locality: Springfield Medical Centre, 110 Union Road, Stockwell,
London SW8 2SH
