Adults between the ages of 40 and 74, who have not already been diagnosed with one of these conditions, can be invited (once every 5 years) to have a check to assess their cardiovascular risk and be offered support and advice to help them reduce or manage that risk.

Description of the procurement
The service will deliver health check-ups for adults aged 40 to 74 in Barnsley. The service will help to prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia. The contract will run from 1st August 2023 for 3 years until 31st July 2026 with the option to extend for two additional years of 12 months each.
Price is not the only award criterion and all criteria are stated only in the procurement documents
