he CCG wishes to build on the successes of the Involving Medway community engagement partnership programme, to improve its ability to measure outcomes and to develop and extend its reach. A provider or a partnership will meet the following needs: 1. A community based engagement programme and a community health researcher network 2. A small grants programme, encouraging individuals to become more connected in their local neighbourhood, addressing loneliness and social isolation 3.
