To work in partnership with East London NHS Foundation Trust (ELFT) as part of the CCGs Integrated Care Programme to enhance ELFT’s existing Telehealth service by supporting development of best practice and provision of services to particular groups that are currently under represented. These will include people stratification based on the recent and number of A and E and GP attendances with relevant LTCs (Long Term Conditions), including those with mental health issues.
