Midlands Partnership NHS Foundation Trust requires the provision of Community Connector Services within South Staffordshire. The Community Connector role shall facilitate and signpost vulnerable individuals to support available within their community. The aim of the role is to be an integral part of the Stay Well Clinics, by supporting people to stay as healthy as possible, manage their care locally and achieve positive health & wellbeing outcomes.
