The Adults' Outreach Service is commissioned to enable people with assessed needs to lead a full life, maximising independence, promoting health and wellbeing and supporting people to remain safe and connected to local communities. The service is designed to support needs, and those with more complex needs, in an innovative way. The Provider will deliver the Service to adults aged 18 and over who have an eligible Care Act 2014 assessed need, identified by the Council’s Assessment Teams. The Service is designed to bridge the gap to support adults in the community who may be socially isolated or disengage with essential services that may cause risk or harm to themselves, due to a mental, physical health condition, neurodiversity or a learning disability. The Service aims to reduce the risk of homelessness, tenancy breakdown, cultural, religious and language barriers, as well as social isolation and loneliness which can lead to a deterioration in an individual’s emotional, mental or physical health and wellbeing. The Provider will work in coproduction with the Individual and their family/ Carers (where consent is given), to support the Individual to be as independent as possible, and enabled to live a “good life” and play an active part in their community. The Service will promote social interaction and relationship building with peers. The Service will address the needs of people holistically, ensuring social, cultural and religious needs of people are acknowledged, addressed and supported.

This is a joint procurement with the Local Authority and the Integrated Care Board. This procurement is for a limited number of Multi-award Contracts on a no guarantee of work basis. The first 20 highest scoring tenderers for each Lot will be awarded a contract for the relevant Lot (provided they pass the quality threshold (see section 17.16 of the Invitation to Tender document). Where there is a tie at 20th position, contracts will be awarded to all 20th placed Tenderers.
Where Tenderers apply for both Lots their responses to each Lot must be identical. For the avoidance of doubt, identical responses to quality questions must be uploaded for both Lot 1 and Lot 2 and not different responses per Lot. The submission of different responses per Lot will result in the responses under Lot 2 being disregarded and the response from Lot 1 being applied and assessed.
