Thank you for logging on to the system to see if you can provide support to the child and their family who require short breaks support in the Willerby area (HU13 9).
The tender deadline is on Friday 4 April 2025 at 12 noon.
Staffing is 2:1, hours requested are term time 6 per week and 6 hours per week in the holiday period.
Please note our young person will have an operation around May 2025, and during the recovery period of 6 – 8 weeks, he will require 2 x hrs per day x 7 days per week to support parent / carer with personal care needs in the mornings.
You will find attached the Lot 2 Personal Care referral document. Within Appendix A there will be the referral information outlining the needs of the child/young person, including when the support is required.
If you are able to meet the needs of this child, please complete ‘Section Two’ within the Lot 2 Personal Care referral and upload to submit your response.
