Thank you for logging on to the system to see if you can provide support to the child and their family who require short breaks support in the Swanland area (HU14 3).
The tender deadline is on Friday 25 October 2024 at 12 noon.
Staffing is 1:1, hours requested are term time 6 per week and 16 hours per week in the holiday period.
The family would prefer weekend if possible, term time however can be flexible.
You will find attached the Lot 1 Non-Personal Care referral document. Within Appendix A there will be the referral information outlining the needs of the child/young person, including when the support is required.
If you are able to meet the needs of this child, please complete ‘Section Two’ within the Lot 1 Non-Personal Care referral and upload to submit your response.
