The principal aim of the service is to support the patient to navigate their admission, stay and discharge from secondary care and transition back into community setting supporting any follow up primary care interventions and improve patient compliance with health interventions therefore reducing inappropriate early discharge and re-admission rates. It will offer support to the individual in health settings and the community by undertaking a comprehensive multidisciplinary assessment of patients’ longer-term needs to address the complex root causes of homelessness and ill health. This will contribute to the overarching aim of the Changing Futures programme, to improve outcomes for adults experiencing multiple disadvantages. Other aims include: • Support the development of an environment conducive to delivering safer, more joined up discharges for the homeless population supporting flow and bed turnover • Help patients have a coordinated discharge plan to transition back into a community setting with all health and social partners kept informed and supporting as required • Embed robust connections across health (primary and secondary), Local Authority Housing Services and VCF organisations • Identify barriers, training, and development needs in relation to rough sleeping, homelessness and discharge planning and develop recommendations that can influence wider learning across the system, including preventative strategies to reduce homelessness.
