PR2023 The London Pathway - Severe And Multiple Disadvantage Needs Assessment and Support Model in the Hospital Environment

A comprehensive assessment of the health and wellbeing needs of people attending hospital in Hull who experience severe and multiple disadvantage (SMD). This will form part of a wider needs assessment for people with SMD in Hull. Recommendations about further development of models of care for people experiencing, or at risk of, SMD in Hull informed by the hospital perspective gained from undertaking the needs assessment.
