Technology Enabled Care - Connected Care Platforms

We are keen to explore the benefits of Connected Care Platforms through a 9 Month trial which will be part of a wider test of change, primarily to support early discharge, at the same time preventing avoidable admissions to Hospital, and enabling people to live independently for a long as possible within their own homes. We are excited about the potential for Connected Care Platforms to support proactive care service interventions, informed through business intelligence which both predicts and prevents incidents such as Falls and early diagnosis of UTIs. We intend to deploy Connected Care both in short term care (Lot 1), and long-term care (Lot 2), to enable us to work collaboratively with a range of stakeholders to optimise the service design in each pathway, and evaluate the benefits to help inform future deployments.
