Thank you for logging on to the system to see if you can provide support to the child and their family who require short breaks support in the Woodmansey - Beverley area (HU17 O).
The tender deadline is on Friday 13 January 2023 at 12 noon.
Staffing is 1:1, hours requested are 4 hours per fornight term time and 4 hours per week in the holiday period.
Request is for sibling 1083186 to be supported at the same time, so each child’s package to be delivered together. This means 2 staff per session.
You will find attached the Lot 1 Non-Personal Care referral document. Within Appendix A there will be the referral information outlining the needs of the child/young person, including when the support is required.
