Thank you for logging on to the system to see if you can provide support to the child and their family who require short breaks support in the Beverley area (HU17 9).
The tender deadline is on Friday 13 January 2023 at 12 noon.
You will find attached the Lot 2 Personal Care referral document. Within Appendix A there will be the referral information outlining the needs of the child/young person, including when the support is required.
Staffing is 1:1, request is for school holiday periods only and ideally 5 hours per day for three days a week.
If you are able to meet the needs of this child, please complete ‘Section Two’ within the Lot 2 Personal Care referral and upload to submit your response.
