Thank you for logging on to the system to see if you can provide support to the child and their family who require short breaks support in the Halsham area (HU12).
The tender deadline is on 22 November 2022 at 5pm.
You will find attached the Lot 1 Non-Personal Care referral document. Within Appendix A there will be the referral information outlining the needs of the child/young person, including when the support is required.
If you are able to meet the needs of this child, please complete ‘Section Two’ within the Lot 1 Non-Personal Care referral and upload to submit your response.
Please note this is an emergency search and the young person does have personal care needs which parent/carer will be responsible for.
