Good afternoon,

Thank you for logging on to the system to see if you can provide support to the child and their family who require short breaks support.

The tender deadline is Monday 3 October 2022 at 12 noon.

You will find attached the Lot 2 Personal Care Individual Placement Agreement. Within Appendix A there will be the referral forming outlining the needs of the child/young person, including when the support is required in the Swanland area (HU14 3).

If you are able to meet the needs of this child, please complete ‘Section Five’ within the Lot 2 Personal Care Individual Placement Agreement and upload to submit your response.

Please note, the request is for 6 hours term time, and 16 hours in the holidays with an additional 1 overnight per week in the holidays.

We will consider at this stage, the 6 hours term time, and 16 hours in the holidays with a view to increasing the package to 1 overnight in the holidays.
