Thank you for logging on to the system to see if you can provide support to the child and their family who require short breaks support.
The tender deadline is Friday 24th June, at 12noon.
You will find attached the Lot 2 Personal Care Individual Placement Agreement. Within Appendix A there will be the referral forming outlining the needs of the child/young person, including when the support is required.
If you are able to meet the needs of this child, please complete ‘Section Five’ within the referral form (Appendix A) and upload to submit your response.
