For 2020/21 six Nottingham City practices chose not to deliver the ESDS services. NHS Nottingham & Nottinghamshire CCG is responsible for ensuring that the patients registered at these practices are able to access the services contained within the ESDS. The services are: • Ear Irrigation • Treatment Room services • ECG Monitoring & Interpretation • H Pylori • Phlebotomy: Clinic based (for 12+ years) • Phlebotomy: Domiciliary service • Spirometry This Enhanced Primary Care Services Bundle (Nottingham City) will be provided in a primary community based setting for patients where these services are not provided by their GP practice as a result of non-participation in the ESDS. The six practices have a combined registered raw list size of 38,551 (as at 1st July 2020) with a normalised weighted list size slightly lower at 38,080. The practices are located across the City over four Primary Care Networks (PCNs) – Nottingham City East (1), Radford & Mary Potter (3), Bulwell & Top Valley (1) and BACHS (1). The population from these practices are diverse and multi-cultural, with high rates of deprivation and poor health. A map showing the PCNs within the City is available from the CCG. Objectives The objectives are to: • Improve access to health services in the community, reducing the need for unnecessary hospital attendance or admissions • To provide a timely service to patients registered with a GP Practice within Nottingham City • To provide a service at an appropriate time, location and environment • Provide services in line with the Care Quality Commission standards • Patients are encouraged and supported to self-manage their conditions in the community
